Program details

What this program offers

Our Partial Care program offers an array of services to assist a person on their path to wellness. Admission to our program requires a psychiatric diagnosis and history of crisis intervention and/or higher level of care (i.e. psychiatric hospitalization or psychiatric emergency contact).

Our services include:

  • Psychosocial/Psychoeducational Groups
  • Individual Counseling
  • Medication Education
  • Psychiatric rehabilitation planning
  • Community integration and social skills development

Our members work collaboratively with their counselors to develop a Behavioral Health Care Plan that empowers them to ultimately transition to outpatient services and be successfully integrated into their community.

Group Counseling

Our Group counseling program has a broad clinical and daily-living focus aimed at emphasizing the goals of the whole person towards achieving and maintaining overall wellness:

Symptom management and coping skills

  • Identifying triggers, warning signs, and early symptoms
  • Developing coping and self-soothing strategies
  • Emotion regulation and distress tolerance
  • Anger management
  • Relapse prevention planning

Psychoeducation on mental health and wellness

  • Understanding one's diagnosis and symptoms
  • Medication education, adherence, and communicating with prescribers
  • Understanding the connection between thoughts, feelings, and behaviors
  • Stress management and relaxation techniques

Daily living and independent functioning

  • Personal hygiene and self-care routines
  • Nutrition, meal planning, and healthy habits
  • Money management and budgeting
  • Household and organizational skills
  • Time management and structuring the day

Social and interpersonal skills

  • Communication and assertiveness training
  • Building and maintaining healthy relationships
  • Boundary-setting
  • Conflict resolution and problem-solving
  • Reducing social isolation

Community integration and resources

  • Using community resources and supports
  • Transportation and navigation skills
  • Accessing benefits and entitlements
  • Safety planning and crisis resources

Recovery and personal growth

  • Goal setting and motivation
  • Building self-esteem and self-advocacy
  • Leisure, recreation, and healthy use of time
  • Spirituality/meaning (as appropriate)

Goals commonly targeted

  • Stabilize symptoms and reduce hospitalization/crisis episodes
  • Increase independence in daily living and self-management
  • Improve social functioning and reduce isolation
  • Strengthen coping skills and resilience
  • Support recovery, community integration, and quality of life
  • Develop and work toward individualized recovery goals

Our program is offered Monday through Friday from 9:00 a.m. to 3:00 p.m.
